Snelling Staffing is seeking a compassionate and skilled Home Health Physical Therapist to join our client’s team in Lawrenceville, Georgia (30046). In this role, you will deliver high-quality, personalized physical therapy services directly to patients in their homes. PAY/SCHEDULE/LOCATION/BENEFITS:

• Compensation: $85,000 – $90,000 per year (+ Mileage reimbursement provided)
• Location: Lawrenceville, Georgia (Zip Code: 30046)
• Employment Type: Direct Hire
• Schedule: Standard business hours; see 5-6 patients per day

HOME HEALTH PHYSICAL THERAPIST DUTIES:

• Conduct comprehensive assessments of patients’ physical capabilities, including strength, range of motion, and mobility.
• Develop and implement customized treatment plans based on individual needs and physician recommendations.
• Utilize therapeutic exercises, manual therapy, and other modalities to promote functional independence.
• Educate patients and caregivers on techniques for exercises and daily activities to ensure continued progress.
• Track and document patient progress, adjusting plans as needed to meet therapeutic goals.
• Collaborate with an interdisciplinary team of healthcare professionals to ensure seamless, high-quality care.
• Maintain compliance with state regulations, Medicare guidelines, and organizational policies.

HOME HEALTH PHYSICAL THERAPIST REQUIREMENTS:

• Educational Requirements: Bachelor’s, Master’s, or Doctorate in Physical Therapy from an accredited institution
• Licensure: Current state licensure as a Physical Therapist in Georgia
• Experience: At least 1 year of experience in physical therapy, preferably in home health or rehabilitation settings
• CPR certification is required
• Strong clinical assessment, critical thinking, and communication skills
• Proficiency with electronic medical record (EMR) systems
• Reliable transportation and a valid driver’s license
